Earthlink solution
It's bizarre, but here's what I do. I use Net2Phone's calling-card service, which is disconnected from its PC phone service. Their calling-card is one of the cheapest by far of any I've seen. A 99 cent per month fee is the only service charge. Auto refill. Etc. So what I do when travelling is I use the international calling card number for the country I'm in and I dial a toll US number for Earthlink. Most countries outside US are 10 to 15 cents per minute, typically dialed via a local toll-free number.
